About Consent

BACKGROUND INFORMATION

There must always be consent for services provided by a psychologist. This applies to both adults, children, cohabiting parents, and divorced parents.

On this page, you will find consent forms for services related to both children and adults.

It is crucial to fill out and bring/send these forms in connection with the initial contact at the clinic or for online services.

DIVORCED PARENTS

There must be consent from both the custodial parent and the non-custodial parent for testing, examination, or therapy with private practicing psychologists.

Psychologists in private practice operate under different rules than psychologists in the state and municipal systems. The Danish Act on Parental Responsibility was passed in the Danish Parliament in 2007 and stipulates the requirement for consent regarding services from psychologists.

Every parent with parental authority must provide detailed written consent to receive services in the clinic.

In connection with our upcoming appointment, please print, fill out, and bring the relevant consent form from the list below:

IF THERE IS NO CONSENT

Therapy or examinations will not be initiated without written consent. If the consent form is not signed and brought along, it will be considered a non-appearance, and it will be invoiced according to the applicable rates.
